Situated approximately 90 minutes west of Brisbane, it provides affordable living, excellent schools, beautiful parks and gardens and a strong sense of community.To be eligible for this position you must:Identify as an Aboriginal and/or Torres Strait Islander personHold a Certificate IV in Aboriginal and Torres Strait Islander Primary Health Care (Practice)Be registered as an Aboriginal Health Practitioner or be eligible for registrationHave experience supporting primary healthcare delivery, health promotion and chronic disease managementDemonstrate solid communication and relationship‐building skillsHold a current Open C Class Driver LicenceHold current CPR certificationBe able to obtain a Blue Card and National Police CheckHighly regarded:Experience working within an Aboriginal Medical Service or Community Controlled Health OrganisationExperience using Best Practice softwareExperience supporting 715 Health Checks and care planningExperience in chronic disease management and preventative health programs
