Job Title: Intake Nurse (RN) – NDIS & DVA
Location: Sydney Area but must be willing to travel on different areas such as Wollongong
Job Type: Casual

We are seeking compassionate, reliable, and motivated Registered Nurses (RNs) who are passionate about community nursing and committed to providing exceptional clinical care.
This is for a casual, community-based Intake role (approximately 20 hours per week).
About the Role
This role involves completing intake assessments, providing clinical care to DVA clients, and supporting NDIS participants.
You will work autonomously while being supported by a robust leadership and clinical team.
Key Duties
Conduct initial intake assessments in clients' homesComplete care plans, risk assessments, and clinical documentationUndertake DVA assessments in line with funding and compliance requirementsProvide clinical care including injections, wound care, and diabetes managementConduct regular follow-ups for DVA clients requiring clinical oversightSupport both NDIS and DVA participants to ensure continuity of careEngage with hospitals, GP clinics,



and referral partners to promote our DVA programMaintain accurate and compliant records
About You
Current registration as a Registered Nurse (RN) with AHPRADemonstrated clinical experience within community nursing, aged care, primary health, rehabilitation, or similar healthcare settingsStrong clinical assessment and decision-making skillsExperience in wound care, medication management, chronic disease management, and health monitoringExcellent communication, interpersonal, and documentation skillsAbility to work autonomously while collaborating effectively within a multidisciplinary teamCurrent First Aid & CPR certification
Valid Australian Driver's Licence and access to a reliable, insured vehicleFull Australian working rightsFlexibility to travel between client locations and work a variety of shifts
Highly Regarded
Previous experience providing nursing services to DVA & NDIS clientsExperience within community nursing, home care, or aged careKnowledge of DVA & NDIS community nursing guidelines and funding modelsExperience using electronic clinical documentation systemsCurrent NDIS Worker Screening CheckUnderstanding of person-centred, strengths-based, and culturally safe care practices
