Townsville Aboriginal and Islander Health Service – Townsville QLD2d ago , from Townsville Aboriginal and Islander Health ServiceThis position of Indigenous Health Worker (IHW) is responsible for the delivery of comprehensive primary health care services including sharing informative resources and imparting educational strategies to patients with a view to improving patient health outcomes.
The position is an integral part of the multidisciplinary health care team, particularly in relation to supporting health team communication and advocating on behalf of patients both with internal and external stakeholders to ensure patients access the care and services they need.
The position also involves, but is not limited to the following:Supporting patients in accessing culturally safe medical care within the clinicPerforming clinical duties within the health worker scope of practice including health checks assessments, clinical observations such as blood pressure, performing clinical tests such as ECGs, micro-urines, and point of care testing such as HbA1c and INRContributing to formulation and updates of patient care plansProviding patient education and community health promotionMaintaining clinical rooms to ensure these are fully stocked, checked, clean and tidied on a regular basis daily.Duties & ResponsibilitiesEnsures patient interventions are carried out in an effective and timely mannerConducts a minimum of Five (5) Health Education and billing episodes ******'s)
per weekProcess a minimum of Five (5) Patient Education and billing ******'s) episodes per week, recorded in the Best Practice systemEnsures progress notes, reports and records are completed in the required formatFollows policies and procedures relevant to the work areaPursues professional development opportunities and fulfils registration requirementsPromotes and contributes to a safe, secure environment for staff, patients and visitorsComplies with MBS billing requirements MBS ItemsProvides culturally safe and appropriate health care to meet the needs of the communityProvides health management education to patients in consultation with patientsUndertakes clinical assessments, tests and procedures within appropriate scope of practiceRecognises limitations to scope of practice, calling for help and escalating care where appropriateContributes to the effective formulation and updating of patient care plansMonitors patients' health needs and outcomesProvides Cultural mentoring – support and guidance to non-Aboriginal/Torres Strait Islander colleagues on culturally protected and appropriate patient interactionsParticipates in staff professional development activitiesEnsures consult and treatment rooms are cleaned and stockedFollows infection control protocolsPromotes a safe and harmonious environment for staff, patients, and visitorsApplications Close:Midnight – September 6th
