Your new role:
As a Physiotherapist within Chronic Disease Brisbane South, you will provide high-quality, patient-centred physiotherapy services as part of a multidisciplinary team supporting adults living with complex chronic disease.
- Deliver physiotherapy assessment, intervention and evaluation across the Chronic Disease Service, including cardiac, pulmonary and heart failure rehabilitation, using evidence-based practice and established clinical protocols.
- Manage an allocated caseload effectively, applying clinical judgement, prioritisation and time-management skills while maintaining accurate patient records and service documentation.
- Work collaboratively with patients, carers, families, health professionals and external agencies to support consumer-centred goals and achieve optimal patient and service outcomes.
- Contribute to clinical education, quality improvement, service development and research activities, with opportunities to support students and less experienced staff under the guidance of senior clinicians.
About you:
We are looking for a motivated and collaborative Physiotherapist who is committed to delivering safe, compassionate and evidence-based care and who is keen to develop their clinical expertise within a supportive multidisciplinary environment.
- You have demonstrated ability to provide high-quality, effective and patient-centred physiotherapy assessment and management for people living with chronic disease.
- You communicate effectively, both verbally and in writing, and work collaboratively with an interprofessional team to achieve the best outcomes for patients.
- You are committed to ongoing learning and professional development and demonstrate the ability to build your clinical knowledge and expertise through active learning and participation in professional development.
- You have demonstrated, or can rapidly develop, the knowledge and skills required to contribute to student education and training, as well as local service development and quality improvement initiatives.
