The opportunity
You’ll provide nursing supports across home and community settings in Bundaberg and surrounding areas. This is a role for nurses who want to combine strong clinical standards with genuinely person-centred care.
Depending on your role, scope and experience, you may support people who need routine community nursing, disability supports with a clinical overlay, or more complex home-based care that requires accurate documentation, sound judgement and early escalation.
For Enrolled Nurses, the role focuses on excellent direct nursing care, monitoring, documentation and escalation within scope and direction.
What you’ll do
- Deliver safe nursing care in the home and community in line with each person’s needs, plan and your scope of practice
- Provide clinical supports such as medication administration/support, wound care, diabetes-related supports, continence and catheter care, health monitoring, and other nursing tasks within role and competence
- Identify deterioration, risks or concerns early and escalate through the right pathway
- Contribute to care planning, reviews, handovers and continuity of care
- Work closely with support workers, allied health, families, coordinators and other clinicians
- Complete accurate progress notes, incident documentation and relevant clinical records in a timely way
- Support people to build safety, confidence and independence at home and in the community
What you’ll bring
Essential
- Current AHPRA registration as an Enrolled Nurse
- Current CPR and First Aid, or willingness to refresh before commencement
- Experience in community, disability, aged care, primary care, rehab or complex support environments
- Strong clinical judgement, clear documentation and good escalation discipline
- NDIS Worker Screening Clearance, or ability to obtain
- Blue Card if required by participant cohort
- Current Australian driver licence and reliable vehicle with comprehensive insurance
- Confidence using mobile and digital systems for rostering, notes and care records
- Empathy, professionalism and a genuine commitment to person-centred care
Highly regarded
- Community nursing or NDIS experience
- Experience supporting people with complex disability or chronic health needs
- Medication endorsement for Enrolled Nurses
- Experience with wound care, catheter care, continence support, diabetes care, enteral feeding or other community-based clinical supports
- Availability across weekdays, weekends and after-hours shifts where required

Pay, perks & conditions
