What You'll Be Doing

Person-Centred Care
Deliver tailored care management that supports independence, dignity, and individual choice for older people living at home.

Care Planning & Coordination
Develop, implement, and review personalised care plans in collaboration with participants, families, and service providers.

Stakeholder Engagement
Work closely with clients, families, healthcare professionals, and external providers to ensure coordinated, seamless care.

Budget & Program Management
Manage individual program budgets,



ensuring supports are delivered effectively within funding guidelines.

Monitoring & Review
Regularly assess client outcomes and adjust care plans to meet changing needs.

Documentation & Compliance
Maintain accurate records and ensure compliance with program requirements, standards, and reporting obligations.

Teamwork & Collaboration
Work independently and as part of a multidisciplinary team within a energetic and supportive environment.

What You Bring

• Experience in aged care case management
• Demonstrated ability to manage budgets and funding programs
• Strong communication and relationship-building skills
