About Us

Swan Disability Services provides practical, respectful and person-centred support to people living with disability across Perth.

Our aim is simple — to support people to live as independently as possible, stay connected with their community and work towards the goals that are important to them.

We are looking for reliable and caring Community Support Workers to join our growing team.

About the Role

As a Community Support Worker, you will work directly with NDIS participants in their homes and within the community.

Every participant is different, so your day may involve supporting someone to attend an appointment, go shopping, participate in a community activity, develop everyday living skills or complete tasks around the home.

We are looking for people who are patient, respectful and genuinely enjoy helping others.

Key Responsibilities

Your duties may include:

- Supporting participants with community access and social activities

- Assisting participants to attend appointments, shopping and other activities

- Encouraging independence and development of everyday life skills

- Providing assistance with daily living and household tasks

- Supporting meal preparation and basic domestic activities

- Providing personal care where required

- Supporting participants to work towards their individual NDIS goals

- Providing transport and assistance within the community

- Following individual support plans and participant preferences

- Maintaining clear and accurate progress notes

- Communicating professionally with participants, families and the wider support team

- Promoting participant choice, dignity, privacy and independence

What We Are Looking For

We would like to hear from people who are:

- Caring, patient and respectful

- Reliable and punctual

- Comfortable working independently in the community

- Valuable communicators

- Able to build positive and professional relationships with participants

- Flexible with working hours

- Committed to providing safe and person-centred support

Previous disability, aged care, community services or support work experience is highly regarded.

Essential Requirements

Applicants should have, or be willing to obtain:

- NDIS Worker Screening Check

- Working With Children Check, where required

- National Police Clearance

- Current First Aid and CPR

- Valid Australian driver's licence

- Reliable and insured vehicle

- Appropriate working rights in Australia

A Certificate III or IV in Individual Support, Disability, Community Services or a related qualification is desirable but not essential for suitable applicants.

We offer:

- Flexible working opportunities

- Supportive and approachable management

- A friendly and respectful working environment

- Opportunities to work with a diverse range of participants

- Ongoing opportunities to develop your disability support experience

- Meaningful work where you can make a genuine difference in someone's day
