Casual | From $58.38/hr | Flexible WFH | Caseload-Based Progression to Part-Time/Full-Time ($80,000 p.a.) | Performance IncentivesJoin High Disability ServicesHigh Disability Services Pty Ltd (HDS) is a registered NDIS provider delivering person-centred disability services in Adelaide.We are seeking an experienced Support Coordinator with an established Adelaide NDIS network and an immediately actionable participant/referral pipeline.This role is specifically for an established Support Coordinator who has prospective participant opportunities and can commence with an active caseload from the outset.This is not a position where you commence employment and then begin looking for participants.IMPORTANT – ESTABLISHED PARTICIPANT NETWORK REQUIREDAn established local network and realistic pathway to an immediate Support Coordination caseload are essential requirements.Applicants should already have a reasonable understanding, without disclosing confidential information, of the approximate number of prospective participants within their professional network who may independently choose to engage HDS for Support Coordination.During recruitment, you will be asked to discuss:Approximate number of prospective participants who could commence Support Coordination with HDS.Approximate Support Coordination hours/caseload they would represent.Expected commencement timeframe.Your established Adelaide NDIS/disability-sector networks.Your track record in generating and maintaining participant referrals and service relationships.Do not provide participant names, contact details or confidential information during recruitment.All engagements remain subject to each participant's independent and informed choice and consent and any contractual, confidentiality or professional obligations applying to the applicant.If you do not currently have an established network capable of generating an immediate Support Coordination caseload, this position is unlikely to be suitable for you.HDS is not recruiting someone to commence employment first and subsequently rely upon HDS to source sufficient participants to establish their caseload.Employment Structure & RemunerationCasual commencement – from $58.38 per hourThe successful applicant will initially commence casually from $58.38 per hour, subject to applicable SCHADS Award classification, pay point, qualifications and experience. Applicable Award entitlements will be paid as required.Initial hours will be aligned with the genuine Support Coordination caseload available at commencement. HDS does not guarantee a predetermined number of weekly hours independent of caseload.As you establish and maintain a larger sustainable caseload, your hours can progressively increase:Casual → Increased Hours → Part-Time → Full-Time ($80,000 p.a.,



subject to applicable Award requirements)Progression is caseload-driven, not time-driven. A sustainable caseload may result in rapid progression, while length of employment alone does not guarantee additional hours or full-time employment.Ongoing employment hours and progression depend upon maintaining a commercially sustainable Support Coordination caseload.Performance IncentivesHDS offers additional performance-based incentives for eligible business-development outcomes and sustainable growth of our Support Coordination service.Incentives may apply where the employee's genuine networking/business-development activities result in new participants independently choosing HDS for Support Coordination.Incentives are additional to ordinary wages and do not replace Award entitlements.Participant Choice & Conflicts of InterestHDS also delivers other NDIS services. Performance incentives are not payable for directing, recommending or influencing Support Coordination participants to purchase HDS's other NDIS supports.Support Coordinators must support genuine participant choice and control and appropriately identify and manage actual, potential or perceived conflicts of interest.About the RoleAs a Support Coordinator, you will:Assist participants to understand and implement their NDIS plans.Support informed choice, control, independence and capacity building.Connect participants with appropriate NDIS, mainstream and community supports.Monitor plan utilisation and assist with effective use of NDIS funding.Work collaboratively with participants, families, nominees, allied health professionals, hospitals and other stakeholders.Assist participants and support networks during complex situations and points of crisis.Facilitate participant, family and provider meetings where appropriate.Maintain high-quality case notes, progress reports and NDIS documentation.Maintain accurate compliance and administrative records.Build productive qualified and referral relationships.Contribute to sustainable growth of HDS's Support Coordination service through ethical networking and business development.What We're Looking ForThe successful applicant will demonstrate:An established Adelaide NDIS/disability-sector network.An immediate pathway to a genuine Support Coordination caseload.Proven ability to generate and maintain participant referrals and engagements.Experience as an NDIS Support Coordinator,



preferably 12+ months.Strong knowledge of NDIS processes, operational guidelines, pricing and participant budgets.Experience implementing plans, monitoring utilisation and reporting participant outcomes.Experience collaborating with allied health professionals and other providers.Experience supporting participants transitioning from hospital/home is highly regarded.Excellent written and verbal communication.Strong report-writing, case-note, organisational and time-management skills.Ability to work autonomously and collaboratively.Solid computer skills, including Microsoft applications and Client Management Systems.Flexibility to attend participants' homes, hospitals and community appointments where required.Relevant tertiary qualifications in Disability Services, Community Services, Social Work or a related discipline are highly regarded.Compliance RequirementsValid driver's licence.Current NDIS Worker Screening Check.Working with Children Check where required.Relevant mandatory NDIS worker training/compliance requirements.What HDS OffersCasual commencement from $58.38/hr, classification dependent.Flexible WFH arrangements.Initial hours aligned with your established caseload.Increasing hours as a sustainable caseload develops.Pathway to permanent part-time/full-time employment.Full-time remuneration of $80,000 p.a., subject to applicable Award requirements.Additional eligible business-development incentives.Autonomy in managing your participant portfolio.Support from an established registered NDIS provider.How to ApplySubmit your CV and brief cover letter addressing your Support Coordination experience and existing Adelaide network/immediate caseload capacity.Without identifying participants or disclosing confidential information, please address:Approximately how many prospective participants could reasonably commence Support Coordination with HDS?Approximately what caseload/billable hours would they represent?How soon could those engagements reasonably commence?What established Adelaide NDIS, allied-health, hospital or community referral networks do you maintain?What experience do you have generating and maintaining a sustainable Support Coordination caseload?All participant decisions remain voluntary and subject to informed choice and consent.Please NoteThis vacancy is specifically for an experienced Support Coordinator with an established network and an immediately actionable pathway to a sustainable participant caseload.
