What You'll Be Doing:
Community Case Management: Conduct home reviews, risk assessments,
and individualised care plans under the 2026 Support at Home program. Workforce Support: Guide and supervise local Health Care Workers (HCWs) for safe care delivery.
Multi-Disciplinary Collaboration: Partner with local GPs, Allied Health, and participant families. Budget & Compliance: Monitor funding allocations and maintain documentation (TRAKCare/RiskMan). What
You'll Need to Apply: Qualifications: Degree/Diploma in
Case Management, Social Work, Psychology, Nursing, or Allied Health. Experience: Background in community aged care, CHSP, or NDIS support coordination.
