Ozcare is seeking a compassionate Client Liaison Officer for the Gold Coast Aged Care at Home team on a permanent part-time basis. You will coordinate client care, oversee intake, assessment, care planning and ongoing reviews to ensure a seamless experience for older Australians living at home.
Reporting to the Care Partnering Lead, you will maintain solid client and family relationships, manage documentation, and contribute to quality improvement while complying with policies and standards.
