Gandangara Local Aboriginal Land Council (GLALC)’s main objective, for each of our Members, is to improve and protect their social and emotional wellbeing by creating a meaningful connection between us based on mutual respect and understanding. We provide a range of services, regular open communication, public meetings, information sharing and a culturally secure place for our Members to come together to yarn, connect and feel they belong.
Gandangara Health Services (GHS) - Community Healing Centre is an Aboriginal Community Controlled Health Service Organisation serving the South-Western Sydney. Operating under GLALC, GHS provides a variety of cultural, clinical, and allied primary health services that are grounded in cultural protocols and co-created with the Aboriginal and/or Torres Strait Islander people.
The Opportunity
The Gandangara Community Healing Centre Case Manager is a unique and rewarding position that will make a significant impact on the lives of First Peoples in Southwest Sydney facing substance use issues. This role allows you to apply your expertise in case management to empower individuals on their recovery journeys, fostering their health, well-being and resilience.
Reporting to the Practice Manager, Senior Clinical Lead and Psychologist, you will have the chance to engage in meaningful community work, contributing to the development and implementation of innovative support programs and intervention that respect and honour the cultural heritage of the participants we support. This position not only offers the satisfaction of making a real difference in the lives of individuals and communities but also provides a platform for professional growth and development in the field of community work and substance use recovery. About You
You are a dedicated case manager deeply committed to supporting the wellbeing and recovery of First Peoples dealing with substance use challenges with a strong foundation in culturally sensitive, holistic and trauma-informed care. You possess a profound respect for the cultural heritage and community connection of individuals you serve.
Your expertise includes comprehensive assessments, case management including information, referral and advocacy, and interagency networking. You excel in building rapport with participants and their support networks, facilitating community support approaches. Your approach is collaborative, empathetic, and empowering aimed at enabling individuals to understand their experiences, develop coping strategies and enhance their resilience.
Key Responsibilities
- Provide culturally sensitive, holistic, and trauma-informed counselling services tailored to the needs of First Peoples.
- Provide community support services that are culturally sensitive, holistic and trauma-informed specifically targeted to the needs of First Peoples dealing with drug and alcohol related health conditions in South West Sydney.
- Utilise case management expertise to assess and support drug and alcohol related health conditions among First Peoples ensuring assessments are culturally informed.
- Employ a variety of community support interventions and approaches that respect and incorporate Aboriginal and Torres Strait Islander cultural practices and values, facilitating participants exploration of their experiences, development and tracking of care plans and fostering a sense of resilience.
- Ensure all support interventions and practices adhere to be established best practice standards, protocols and guidelines particularly those relevant to culturally sensitive care for Aboriginal and Torres Strait Islander communities.
- Conduct case management sessions aimed at reducing drug related harm, fostering a supportive environment for recovery and ensuring interventions are aligned with the highest standard of care and ethical practices.
- Provide support as part of a broader treatment plan which may include collaboration with other drug health treatment services ensuring a comprehensive and holistic approach to recovery with a focus on empowering participants to reach their full potential.
- Work with a multidisciplinary team including Psychologist, Counsellor, Aboriginal Health Worker, Community Liaison Officer and Intake Officer to ensure the provision of safe effective and culturally sensitive and person-centred care.
- Collaborate with key stakeholders including psychiatrists, psychologists and Allied Health Professionals to facilitate holistic care and address conflict participant needs. This includes collaboration with South West Sydney Local Health District, Department of Housing, Department of Corrective Services and other services and community organisations to ensure alignment with broader health strategies and initiatives
Qualifications
- Possessed qualifications in case management, substances use or a related field, or equivalent experience
- Demonstrate strong expertise and experience in providing case management within Aboriginal and Torres Strait Islander communities using culturally informed and evidence-based approaches
- Show a deep understanding of evidence-based practices, clinical care standards in AOD treatment, and adherence to relevant legislative requirements and ethical guidelines
- Exhibit a background in delivering high quality person-centred, trauma-informed care with a focus on measurable outcomes and recovery focused interventions
- Demonstrated experience working across cultures and in particular with Aboriginal and Torres Strait Islander communities
- Excellent communication and interpersonal skills, capable of engaging effectively with a diverse range of individuals and groups.
- Current NSW Drivers Licence and willingness to engage with local communities as needed.
- Compliance with necessary background checks, including a National Police Check and a NSW employee Working with Children Check clearance.
