The Clinical Nurse at Distinction at Home Care is responsible for providing comprehensive clinical assessment, care planning, care coordination, and ongoing monitoring of clients receiving services in the community. This includes older persons receiving Aged Care Services, National Disability Insurance Scheme (NDIS) participants, Department of Veterans' Affairs (DVA) clients, and privately funded clients. The Clinical Nurse delivers person-centered, evidence-based nursing care that promotes health, wellbeing, independence, dignity, safety, and quality of life. The role provides clinical leadership, supports secure service delivery, and contributes to the organization's clinical governance framework. The Clinical Nurse reports to the Clinical Care Manager and works collaboratively with clients, families, support workers, allied health professionals, medical practitioners, and external stakeholders.

Key responsibilities

- Conducting comprehensive nursing assessments within the client's home environment
- Identifying health concerns, clinical risks, support needs, and goals of care
- Developing, implementing, reviewing, and evaluating individualized care plans




- Undertaking risk assessments including falls risk, skin integrity, continence, medication management, manual handling, and environmental safety
- Delivering nursing care within clients' homes and community settings
- Wound assessment and management
- Medication management and administration
- Catheter and continence management
- Chronic disease management and monitoring
- Delegating care activities in accordance with NMBA standards and organisational policies

About you

- Current registration as a Registered Nurse with AHPRA
- Minimum 3 years' post-registration nursing experience
- Current unrestricted Australian Driver License and reliable vehicle
- Current National Police Check
- NDIS Worker Screening Clearance
- Current First Aid and CPR certification
- Demonstrated knowledge of community nursing, aged care, disability support, chronic disease management, and palliative care
- Sound understanding of Aged Care Quality Standards, NDIS Practice Standards, DVA Community Nursing requirements, and relevant legislation
