Centacare is seeking an experienced Clinical Care Partner – Registered Nurse to join our Aged Care team in Darra, Brisbane South. This full-time role supports older Australians to stay secure, healthy and independent at home, reporting to the Coordinator - Aged Care Services.
You will perform clinical assessments, care planning, wound care and medication management, coordinate care with families and multidisciplinary teams, and mentor Care Partners to ensure high-quality, person-centred care.
