Practice Information

This Aboriginal Community Controlled Health Organisation provides culturally safe, comprehensive primary health care and wellbeing services to Aboriginal and Torres Strait Islander peoples and the wider community across North West Queensland.
As a multidisciplinary group practice, the team includes General Practitioners, Nurses, Aboriginal Health Practitioners, Aboriginal Health Workers, Allied Health Professionals, and Social and Emotional Wellbeing staff working collaboratively to deliver patient-centred care.
Services include general medical consultations, chronic disease management, health assessments, immunisations,



mental health and social and emotional wellbeing support, family wellbeing services, nursing care, allied health services, care coordination, and health promotion programmes.

Position Description

This organisation is seeking a Social Worker to join its multidisciplinary team, providing client-centred social work services to individuals, families and communities.
The role focuses on enhancing social and emotional wellbeing through assessment, advocacy, case management and therapeutic interventions. Working within a trauma-informed and culturally secure framework, the Social Worker will collaborate closely with internal programs and external stakeholders to address the social determinants of health and support holistic care outcomes for clients.

Key Responsibilities

- Providing evidence-based and culturally appropriate social work services that support the social, emotional and practical wellbeing of clients and families

- Advocating for improved access to health, housing, education, legal and community services

- Strengthening client capacity, resilience and self-determination through supportive, respectful and collaborative practice

Position Requirements

- Degree in Social Work or another qualification relevant to social work practice

- Membership (or eligibility for membership)



with the Australian Association of Social Workers (AASW)

- Mental Health Accreditation

- Experience in assessment, case management, client advocacy or community support

- Understanding of trauma-informed practice and holistic wellbeing approaches

- Strong communication and relationship-building skills

- Ability to work independently and within a multidisciplinary team

- Current C Class Driver's Licence

- Willingness to travel throughout remote and regional communities

- Experience working with Aboriginal and Torres Strait Islander peoples and communities

- Experience in community services, primary health care, or social and emotional wellbeing programs

- Current Queensland Blue Card or willingness to obtain

- Valid authority to work in Australia

After Hours Requirements

Yes. After-hours work may be required during outreach activities or in response to client needs.
