About the role
- Deliver holistic social, emotional, & advocacy support to First Nations patients and their families
- Conduct culturally safe assessments & drive meaningful engagement to support healthcare decisions
- Collaborate with multidisciplinary teams, participate in family conferences, and coordinate discharge planning
- Help foster robust partnerships with local Aboriginal community health providers and external services
- Serve as a cultural resource for hospital staff while supporting health promotion & improvement initiatives
About you
- Identify as Aboriginal and/or Torres Strait Islander
- Experienced in supporting First Nations patients and their families in a healthcare setting
- Strong understanding of the healthcare challenges facing First Nations patients and families
- Excellent communication & teamwork skills coupled with the ability to manage competing priorities
- Have or be willing to undertake a minimum Certificate III in Aboriginal Primary Health Care
- Experience in analysing patient and service metrics, desirable
