Bayside Health - Alfred Care Group is seeking a Social Worker for the HIV Outward Program. You will provide psychosocial assessments and counselling to people living with HIV across inpatient, outpatient, and community settings, collaborating with nursing, medical, and mental health teams to plan and implement timely, quality care.

The role includes outreach clinics, discharge planning involvement, and a patient-directed approach to care.
