Heathcote Health is seeking a compassionate Social Worker to deliver services across acute, aged care and community settings in the Heathcote region. You will assess, counsel, advocate and plan care with a multidisciplinary team to improve health outcomes for clients, families and carers.
The role requires a Social Work qualification, AASW eligibility, current checks, a valid driver licence and experience with older adults and multidisciplinary teams for person-centred care.
