Centred Care Nursing Agency is seeking Community Workers for our various homes around Sydney and Coastal Areas. This is a Full-time role.

You'll be working with people with a range of mental and physical disabilities - supporting them to engage with their community, grow their independence, and live a life they value.

About the role:

- Identifying specific challenges through client interviews and tailoring support to meet individual need.

- Planning, developing and implementing educational, training and support programs tailored to client needs.

- Monitoring and reporting on clients progress.

- Conducting through assessments to determine the type of additional support required to address their challenges effectively.

- Assessing community requirements for client's health, welfare, housing, employment, training and other services.

- Developing and Submitting detailed proposals to secure relevant funding for programs and initiatives.

- Collaborating with stakeholders to ensure all documentation supports ongoing funding and program sustainability.





- Delivering education and care for disabled persons in adult service units and group homes.

- Engaging with families, welfare agencies, government organisations, and private businesses to identify and address pressing community issues.

- Raising awareness about the resources and services available to individuals and families within the community.

About You:

- Passionate about inclusion, empowerment and community participation.

- Experience in disability, mental health or youth support is highly regarded.

- Great interpersonal skills and professional boundaries.

- Comfortable with personal support and adaptable routines.

- Confident in working with clients from diverse and socioeconomically disadvantaged backgrounds.

- Driver's licence, reliable vehicle, and confident with technology.

Essential Requirements:

- First Aid and CPR

- NDIS Workers Screening Check

- Working with Children Check

- NSW Police Check or National Police Check

- Certificate IV or Diploma in a relevant field.

- 2 years relevant experience
