The Service Coordinator is responsible for coordinating the delivery of quality care and support services to clients across Caring Your Life's service portfolio. The role focuses on client scheduling and rostering, workforce coordination, subcontractor engagement, client communication, incident escalation, and ensuring continuity of services. The Service Coordinator acts as the primary point of contact for clients and support workers, ensuring services are delivered safely, efficiently, and in accordance with organisational policies, funding requirements, and client preferences.

We prefer bilingual languages: Chinese, Cantonese and more

Key responsibilities

Develop, maintain and manage client service rosters

Match clients with suitable support workers based on skills, experience, availability, cultural preferences and client needs

Ensure services are scheduled and delivered according to approved care plans and service agreements

Provide regular communication and support to clients and their representatives

Coordinate support worker schedules and availability

Manage daily roster changes and service adjustments





Arrange replacement staff when workers are unavailable due to illness, leave, emergencies or other circumstances

Respond promptly to client enquiries and requests

Receive and document complaints, concerns, and feedback

Escalate complex complaints, safeguarding concerns, and unresolved matters to the Head of Operations

About you

Experience in rostering, scheduling, or service coordination within community services, disability, aged care, or health sectors

Solid organisational and time management skills

Excellent verbal and written communication skills

Ability to manage competing priorities in a fast-paced environment

Strong problem-solving and conflict-resolution abilities

High-level computer literacy and ability to use client management systems

Ability to work independently and collaboratively within a team

Understanding of client-centred service delivery principles

Experience working within NDIS, Aged Care or Support at Home programs

Knowledge of workforce scheduling systems and CRM/client management software
