Central Coast Local Health District is seeking a compassionate Social Worker at Gosford Hospital to support people living with chronic kidney disease and their families. You will work within the Renal Service, collaborating with a multidisciplinary team to provide patient-centred psychosocial care and coordinate services across inpatient and outpatient settings.

You will undertake assessments, counselling, advocacy and case management, participate in on-call rosters, and contribute to service
