NSW Outback Division of General Practice (ODGP) provides primary health care services across the outback shires of Walgett, Brewarrina, Bourke, and Cobar, with a strong focus on early intervention, chronic disease prevention and management, and health promotion. We work to improve access to quality health care and outcomes in some of the most remote and disadvantaged communities in NSW.
NSW Outback Division of General Practice (ODGP) is seeking to appoint a Diabetes Educator to a full time position based in Cobar, with outreach to Nyngan.
About the role:
• Provide evidence-based diabetes education and self-management support to people aged 15 years and over
• Deliver individual education sessions in general practice
• Work closely with GPs and multidisciplinary teams to support coordinated, client-centred care
• Support clients with blood glucose monitoring, medications (including insulin), lifestyle modification and complication prevention

A market-competitive salary package ranging from $58 to $65 per hour (negotiable based on skills and experience) is offered under the Health Professional and Support Services Award, plus superannuation, 5 weeks annual leave and leave loading, paid Christmas closure and salary packaging up to $16,050 per annum. Rental subsidy and eligibility to apply for the RDN Relocation Grant are available for the successful applicant.
