Uniting Vic.Tas in Morwell, VIC, is seeking a Senior Assessment and Facilitation Officer to contribute to the Home and Community Care Program for Younger People. You will supervise, consult and support staff within the HACC PYP team while delivering high-quality assessments and care planning.
