Ballarat Hospice Care Inc is seeking a compassionate and skilled Supportive Care Advisor to join our multidisciplinary team providing specialist, home-based palliative care in the Ballarat region.
You will undertake assessments, provide psychosocial and bereavement support, contribute to care planning, and work closely with clinical staff and volunteers to support holistic, person-centred care across our services. Some out-of-hours activities may be required.
