About the role:
Centacare is seeking an experiencedClinical Care Partner – Registered Nurseto join our Aged Care team based inDarra, Brisbane South.This is afull-time, Monday to Fridayopportunity for a Registered Nurse who is passionate about delivering person-centred care and supporting older Australians to remain safe, healthy and independent in their own homes.Reporting to the Coordinator - Aged Care Services, you will use your clinical expertise to assess needs, coordinate care and support positive health outcomes.
Key Responsibilities:
Complete clinical assessments, care planning, reviews and care coordination.
Provide clinical support including wound care, medication management and dementia support.
Monitor client health and wellbeing, escalating concerns and supporting timely interventions.
Promote wellness, reablement and restorative approaches that maximise independence.
Collaborate with clients, families, carers and multidisciplinary teams to achieve positive outcomes.
Provide clinical guidance and support to Care Partners and Support Workers.
Maintain accurate documentation and contribute to quality improvement and clinical governance activities.
Support compliance with aged care standards, policies and regulatory requirements.
Collaboration and support clinical-related investigations, including Reliable Incident Response Scheme (SIRS) requirements

Skills and Experience:
Experience inaged care, community nursingor a similar clinical environment
Demonstrated experience inclinical assessment, care planning and clinical decision-making
Robust understanding ofperson-centred care, clinical governance and multidisciplinary practice
Excellentcommunication, relationship-building and cultural competency skills
The ability towork autonomouslywhile contributing positively to a collaborative team
A genuine commitment to supporting older people to maintain their independence, dignity and quality of life
Experience supporting people living withdementia, palliative or end-of-life care needs, or complex health conditionswill be highly regarded
Experience withRestorative Care, SIRS, clinical governance, quality improvement, mentoring or providing clinical guidance to team memberswill be highly regarded
Familiarity withMy Aged Care and digital care management systemswill be advantageous
Essential Requirements:
Bachelor of Nursing
CurrentAHPRA registrationas a Registered Nurse
Currentdriver's licence
Reliable, registered and comprehensively insured vehicle for work purposes
Current paidBlue Card
Current paidYellow Card / NDIS Worker Screening Clearance
CurrentFirst Aid and CPR certification
Australian Federal Police Criminal History Check
Evidence of the right to work in Australia
