Permanent full-time position
Monday to Friday role – no weekends, nights or shift work

As an Aboriginal Health Practitioner, you will play an important role in supporting the delivery of primary healthcare services, chronic disease management programs, preventive health initiatives and community engagement activities.
Working closely with GPs, Nurses, Allied Health Professionals and other healthcare staff, you will assist clients to navigate the healthcare system, access services and achieve improved health outcomes.

To be eligible for this position you must:
Identify as an Aboriginal and/or Torres Strait Islander person
Hold a Certificate IV in Aboriginal and Torres Strait Islander Primary Health Care (Practice)
Be registered as an Aboriginal Health Practitioner or be eligible for registration
Have experience supporting primary healthcare delivery, health promotion and chronic disease management
Demonstrate strong communication and relationship‐building skills
Hold a current Open C Class Driver Licence
Hold current CPR certification
Be able to obtain a Blue Card and National Police Check
Highly regarded:
Experience working within an Aboriginal Medical Service or Community Controlled Health Organisation
Experience using Best Practice software
Experience supporting 715 Health Checks and care planning
Experience in chronic disease management and preventative health programs
