Support Worker - NDIS In-Home and Community Care

About the Role
As a Support Worker with Royal Health Ability Services, you will assist NDIS participants with daily living activities in their own homes and in the community.
You will work closely with participants and their families to deliver supports in line with each person's individual NDIS plan and support goals.
You will be matched with participants based on your skills and experience.
Our team includes workers supporting participants across a range of ages and support needs, and we take care to find the right fit for both workers and participants.
Key Responsibilities
Assisting with personal care including showering, dressing, grooming, and hygiene
Supporting meal preparation and mealtime assistance in line with the participant's dietary needs and care plan
Medication assistance and administration as directed by the participant's medication management plan
Supporting mobility and transfers using appropriate aids and equipment, in line with the participant's handling plan
Accompanying and supporting participants in the community - appointments, recreational activities, and social outings
Supporting participants to engage in therapeutic, recreational, and social activities aligned with their goals
Completing accurate and timely shift notes and documentation in the care management system
Communicating respectfully and effectively with participants, their families, and the broader support team
Recognising and responding to changes in a participant's health, behaviour, or wellbeing, and escalating concerns appropriately
Following individual care plans, risk management plans, and organisational policies and procedures at all times
Certificate III in Individual Support (Disability), Community Services, or equivalent - or demonstrated relevant experience
Current NDIS Worker Screening Clearance (or willingness to obtain before commencing)
Current First Aid and CPR certificate
Current Queensland driver's licence and access to a reliable, comprehensively insured vehicle
Genuine commitment to person-centred, rights-based disability support
Good written and verbal communication skills
Ability to work independently and as part of a team
Reliable, punctual, and professional with a compassionate approach
Experience supporting participants with disability in a home or community setting
Manual handling and assistive technology competency
Experience with medication administration
Familiarity with NDIS practice standards and participant rights
Experience working alongside family carers in a home environment
Willingness to undertake further training as required for specific participant placements
