NSW Health Aged Care Assessment – Hunter is seeking a Non-Clinical Assessor to perform home support assessments for older people across the Hunter region using NSAF and IAT to identify appropriate supports.
You will develop an individualised Support Plan with goals and services, considering workplace, independence and social factors.
This role requires autonomous field work within a multidisciplinary team, strong time management, and excellent documentation across mobile devices.
