About the role:
You will deliver home-based Occupational Therapy services in the Swan Hill region for clients aged 65+ accessing services through their Support at Home (SAH) package. The role focuses on functional independence, environmental modification, equipment prescription, and reablement.
This is an ongoing permanent position with full-time or flexible FTE options, including family‐friendly hours and varied working days.
You will receive regular one‐to‐one supervision and support from our Practice Lead. You'll also be supported by a large team of OTs, the Allied Health scheduling team, the Operations Manager, and local on‐site staff.

About you:
You are an Occupational Therapist with strong clinical reasoning skills, a client‐centred approach, and a commitment to continuous learning. You are motivated to deliver high-quality care, adapt to varied client needs, and contribute to a positive team culture. You value evidence‐based practice and support older adults to maximise independence, safety, and wellbeing.
We are looking for individuals with:
Recognised tertiary qualification in Occupational Therapy
Current registration with AHPRA
Experience in Community Aged Care with clients receiving HCP/SAH services
Demonstrated knowledge of clinical practice working with a variety of health conditions in 65+ year old clientele
The ability to work autonomously and manage a caseload independently
Have full Australian working rights and are able to pass a national police clearance
Have a valid driver's license and access to a car

A current Working With
Children Check card
Current and ongoing flu vaccinations
Ability to pass a national police clearance and NDIS Clearance
