The IBD CNC is an integral role within the IBD MDT responsible for providing high quality coordination of care for patients with IBD.
This is a fixed term, part time position (5 days/40 hours per fortnight).
A regular day will see you:
Provide comprehensive patient support through the IBD helpline and email service
Use your advanced clinical skills to triage and manage patient enquiries, initiate care plans, and escalat e complex issues as required
Coordinate care for patients on advanced therapies, including scheduling infusions, relevant investigations, and preventative health surveillance
Attend weekly IBD clinics, providing expert advice, education, and support to patients and families throughout all stages of care
About you
You will be someone who has excellent communication skills, with a strong commitment to excellence in patient care.
You'll bring energy and drive to your work, and have a keen interest in developing your skillset.
To be successful in this role you'll bring with you:
Experience working in a senior clinical role (CNS or above)
Excellent organisational,
time-management skills and attention to detail
A flexible, innovative team-orientated approach to service delivery
Prior experience in IBD and/or management of patients who are immunosuppressed will be highly regarded
