Job Title: Intake Nurse (RN) – NDIS & DVA
Location:
Sydney Area but must be willing to travel on different areas such as Wollongong

We are seeking compassionate, reliable, and motivated Registered Nurses (RNs) who are passionate about community nursing and committed to providing exceptional clinical care. This is for a casual, community-based Intake role (approximately 20 hours per week).
About the Role
This role involves completing intake assessments, providing clinical care to DVA clients, and supporting NDIS participants. You will work autonomously while being supported by a strong leadership and clinical team.
Key Duties
Conduct initial intake assessments in clients' homes
Complete care plans, risk assessments, and clinical documentation
Undertake DVA assessments in line with funding and compliance requirements
Provide clinical care including injections, wound care, and diabetes management
Conduct regular follow-ups for DVA clients requiring clinical oversight
Support both NDIS and DVA participants to ensure continuity of care
Engage with hospitals, GP clinics,
and referral partners to promote our DVA program
Maintain accurate and compliant records
About You
Current registration as a
Registered Nurse (RN) with AHPRA
Demonstrated clinical experience within
community nursing, aged care, primary health, rehabilitation, or similar healthcare settings
Strong clinical assessment and decision-making skills
Experience in wound care, medication management, chronic disease management, and health monitoring
Excellent communication, interpersonal, and documentation skills
Ability to work autonomously while collaborating effectively within a multidisciplinary team
Current First Aid & CPR certification
Valid Australian Driver's Licence and access to a reliable, insured vehicle
Full Australian working rights
Flexibility to travel between client locations and work a variety of shifts
Highly Regarded
Previous experience providing nursing services to DVA & NDIS clients
Experience within community nursing, home care, or aged care
Knowledge of DVA & NDIS community nursing guidelines and funding models
Experience using electronic clinical documentation systems
Current NDIS Worker Screening Check
Understanding of person-centred, strengths-based, and culturally safe care practices
