We are looking for an experienced Support Coordinator with Level 2 and/or Level 3 Specialist Support Coordination experience .
Most of the role can be performed remotely from Perth, with the option to work a mix of at-home and the Perth office.
Occasional travel to the South West will be required, expected to be around monthly.
This role will suit someone who is confident working with complex participants, enjoys building strong relationships and solving problems, and is passionate about advocating for positive, participant-led outcomes.
You will manage your own portfolio of participants while working closely with families, guardians, therapists, service providers, community organisations and the NDIA to coordinate high-quality supports that place the participant at the centre of every decision.
What You'll Do
· Provide high-quality Level 2 Support Coordination and/or Level 3 Specialist Support Coordination.
· Work closely with participants, families and support networks to understand individual needs, goals and preferences.
· Support participants to understand, implement and make effective use of their NDIS plans.
· Coordinate supports for participants with complex needs and multiple service providers.
· Identify barriers, risks and gaps in supports and work collaboratively to develop practical solutions.




· Build strong working relationships with allied health professionals, service providers, guardians, government agencies and other stakeholders.
· Support participants through plan reviews, changes in circumstances and other NDIS processes.
· Advocate for participants' rights, choices, independence and preferences.
· Build participant and family capacity to understand and navigate the NDIS and broader service systems.
· Maintain accurate case notes, reports and documentation in accordance with NDIS requirements and Hera Support policies.
· Remain informed about changes to the NDIS and relevant legislation, policies and practice requirements.
What We're Looking For
We are looking for someone who brings:
· Demonstrated experience delivering Level 2 Support Coordination and/or Level 3 Specialist Support Coordination.
· Experience supporting people with complex disabilities, psychosocial needs or complex support environments.
· Strong knowledge and understanding of the NDIS and the broader disability sector.
· Excellent communication and interpersonal skills, with the ability to build trust with participants, families and stakeholders.
· Strong assessment, problem-solving and case management skills.




· Confidence managing competing priorities and working autonomously.
· A collaborative approach when working with multidisciplinary teams and external providers.
· A robust commitment to participant choice, control, dignity and independence.
· High-quality written documentation and report-writing skills.
· A current driver's licence and access to a reliable vehicle.
A relevant qualification in social work, nursing, psychology, community services or a related discipline will be highly regarded, particularly for applicants interested in Specialist Support Coordination.
