cohealth is seeking a part time, fixed term role until July 2027 to support Aboriginal and Torres Strait Islander people with chronic health conditions within the Integrated Team Care Program. The role is based across the North and West of metropolitan Melbourne and focuses on culturally responsive care coordination.

Key duties include liaising with DFFH and services on clients' behalf, assisting with appointments and cultural supports, delivering food relief to the community, and undertaking
