With a dedicated team of experienced and compassionate professionals, Community Care Hub Australia provides a range of disability services, including in-home supports, community access, Supported Independent Living (SIL), Short-Term Accommodation (STA/Respite), Individualised Living Options (ILO), and support for participants with complex needs.

About the Role

Community Care Hub Australia Pty Ltd is currently seeking dedicated and compassionate Disability Support Workers for an immediate start across the Greater Western Sydney region.

As a Disability Support Worker, you will play an important role in supporting participants to achieve their individual goals, develop independence and actively engage with their local communities.

You will provide personalised, respectful and person-centred support to individuals with a range of disabilities and support needs, including:

- ADHD
- Autism Spectrum Disorder (ASD) – Levels 1, 2 and 3
- Acquired Brain Injury (ABI)
- Global Developmental Delay (GDD)
- Multiple Sclerosis (MS)
- Down Syndrome
- Intellectual and psychosocial disabilities




- Other complex and individual support needs

What You'll Be Doing

Your responsibilities may include:

- Assisting participants with daily living activities, including personal care, meal preparation and household tasks
- Supporting participants to attend appointments, social activities and community engagements
- Encouraging independence, skill development and achievement of individual goals
- Providing respectful and person-centred care in accordance with each participant's individual support plan
- Advocating for participants' needs, choices, rights and dignity
- Completing accurate progress notes, incident reports and other required documentation
- Providing feedback and relevant updates to Service Coordinators and other stakeholders
- Working collaboratively with families, allied health professionals and other support servicesAssisting with the implementation of individualised care and support plans
- Responding appropriately to changing participant needs and behaviours
- Following all relevant organisational policies, procedures, NDIS Practice Standards and workplace health and safety requirements

What We're Looking For

To be successful in this role, you will ideally have:

- Certificate III in Individual Support, Disability Support or a related qualification (preferred)




- Experience providing person-centred care and support to people with disabilities
- Strong communication and interpersonal skills
- The ability to build positive, professional and trusting relationships with participants
- A genuine passion for supporting people to achieve greater independence and quality of life
- Patience, empathy and a respectful approach to supporting others
- Strong problem-solving skills and the ability to respond effectively to challenges
- The ability to work independently as well as collaboratively within a team

Essential Requirements

- Current National Police Check
- Current First Aid and CPR Certificate
- Current Working with Children Check (WWCC)
- Current NDIS Worker Screening Check
- Completed NDIS Worker Orientation Module: "Quality, Safety and You"
- Current unrestricted Australian driver's licence
- Access to reliable and insured transportation
- Full Australian working rights
