Ballarat Hospice Care Inc is seeking experienced Specialist Palliative Care Nurses and Registered Nurses to join our home-based palliative care team.
You will provide community-based palliative care through nursing assessment, care planning, symptom support, clinical review and coordinated care. You will work as part of a dedicated interdisciplinary team and contribute to clinical handover, documentation, quality improvement and reflective practice.
