Reporting to the Client Services Manager with direct responsibilities to the Program Supervisors, the Disability Support Worker – Key Worker plays a vital role in empowering individuals with disabilities to live independently, safely, and with dignity. This role delivers personalised, person-centred support that promotes physical, emotional, and social wellbeing, while fostering inclusion and community engagement. The Key Worker is responsible for enhancing each participant's quality of life by supporting their daily living activities, facilitating meaningful community participation, and helping them achieve their individual goals in alignment with their care plans and personal preferences. Through robust relationships, proactive communication, and a commitment to best practice, the Key Worker ensures that each participant receives consistent, respectful, and empowering support. The Key Worker plays an integral role in the custodianship of participants by working closely and collaboratively with participants, their families, carers, support coordinators, healthcare, and allied health professionals as the key point of contact at Carinya.

Key responsibilities

- Develop, implement, and regularly review individual support plans in collaboration with participants, their families or carers, and relevant professionals on an annual basis at minimum
- Ensure all plans are person-centred, culturally responsive, and aligned with the participant's goals, preferences, and NDIS frameworks
- Maintain consistent communication with participants and relevant others to collaboratively monitor progress, identify changes in needs, and adjust supports accordingly
- Coordinate with external providers, therapists, and healthcare professionals to ensure integrated and cohesive service delivery
- Assist in the delivery of structured programs and group activities such as art, music, cooking, fitness, and life skills




- Provide respectful and dignified support with personal care tasks including toileting, hygiene, and continence management as required
- Assist with mobility and transfers, including the safe use of hoists and mobility aids
- Safely and effectively support participants during mealtimes, including feeding assistance, modified diets, and implementation of mealtime management plans
- Safely and effectively support individuals in managing behaviours of concern in accordance with their Behaviour Support Plans
- Accurately record attendance, support provided, hazards, incidents, current allied health plans, NDIS goals and daily notes in line with organisational procedures

About you

- Previous experience as a Key Worker
- Minimum 3 years of experience in the Disability Sector as a Support Worker
- Certificate 3 or higher in disability field
- Current Level 2 First Aid and CPR certification
- Current NDIS Workers Screening Check
- Current Australian/Victorian Police Check
- Successful completion of the NDIS Workers Orientation Module
- Exceptional written and verbal communication
- Knowledge and understanding of relevant legislation and NDIS guidelines

Carinya delivers centre-based and community, individual and group day programs for more than 100 participants from across the northern sweep of Melbourne, as well as community participation and Specialist Disability Accommodation (SDA) supporting residents through Supported Independent Living (SIL).
