About the role As the Clinical Nurse Consultant (CNS)
- Integrated Chronic Care for Aboriginal People (ICCAPP), you will provide high-level clinical expertise, leadership and consultancy to improve access to culturally appropriate chronic care services across the Mehi area.
The goal of ICCAPP is to improve access to culturally appropriate chronic care services for Aboriginal and Torres Strait Islander people aged 15 years and over who have, or are at risk of cancer, diabetes, cardiac, respiratory, stroke and-or renal disease. You will work in partnership with an Aboriginal Health Practitioner dedicated to ICCAPP and collaborate with clients, health professionals and community services to support person-centred and culturally appropriate care.
About you
We are looking for an experienced and highly skilled clinician with a background in chronic care and-or diabetes who is passionate about improving health outcomes for Aboriginal and Torres Strait Islander people. You will bring:
- Current Authority to Practise as a Registered Nurse with AHPRA.
- At least 5 years full time equivalent post-registration experience, with at least 3 years full time equivalent experience in the relevant specialty field.
An approved postgraduate nursing qualification relevant to the specialty. Demonstrated clinical leadership and consultancy skills, with the ability to role model best practice. Demonstrated ability to work effectively in partnership with clients, Aboriginal health services, primary healthcare providers and other services.
A current NSW Driver Licence and willingness to travel regularly across the Mehi area is required.
