Practice Information
Based in Goondiwindi, reporting to Practice Principals and Practice Manager. The team will support people in the local community living with severe and complex mental health, psychosocial and behavioural needs, the people who often fall through the gaps because of distance, complexity or fragmented services. The program delivers intensive, recovery-oriented support for people who face barriers to traditional mental health care: geographic isolation, complexity of presentation, service fragmentation, or co-occurring challenges. Based inside an existing medical centre, it brings outreach, multidisciplinary collaboration, care coordination and integrated care together with local health, social and community services, so people who've struggled to access support can actually get it.
This is a shared-care model. Patients come to the team through a coordinated pathway, and your clinical practice is your own: your assessments, your plans, your judgement. But you're never carrying it alone. You'll work closely with patients' GPs and the wider practice team, and with hospitals, primary care, crisis services and other agencies through case conferencing, coordinated response planning, and integrated care for people with co-occurring mental health, physical health, behavioural, housing and social support needs.
Position Description
You’ll manage a complex and varied caseload, delivering discipline-specific care while working collaboratively within a general practice environment. Day to day, that looks like:
Assessment and clinical intervention
- Comprehensive screening, assessment, care planning, therapeutic intervention and care coordination for people with severe and complex mental health and psychosocial presentations.
- Identifying clinical deterioration, risk escalation, safeguarding concerns and urgent support needs,
and escalating or de-escalating care appropriately across stepped-care or acute services.
- Assertive engagement approaches to improve continuity of care and service access, including for patients at elevated risk or vulnerable to disengagement.
Multidisciplinary and collaborative practice
- Participating in multidisciplinary case conferencing, risk management and coordinated response planning.
- Building working relationships with hospitals, primary care, community services, crisis services and external agencies.
- Interagency collaboration is part of the job, not an occasional add-on.
Outreach and clinic hub facilitation
- Community-based appointments and home visits, and facilitating appointments at outreach clinics, so patients who can't attend independently still get equitable access.
- Delivering care in non-traditional environments where that's what continuity of care needs.
Documentation, reporting and supervision
- High-quality progress notes and documentation, including the standard outcome measures (K10 and MHQoL), which feed into the PMHC-MDS as part of the program's funding and reporting obligations.
- Overall program data reporting through RHealth.
- Gathering relevant information for psychiatrist review and multidisciplinary planning, and contributing to quality improvement and service development.
- Ongoing professional supervision in line with your discipline's and the organisation's requirements, and adherence to trauma-informed, recovery-oriented, culturally safe and person-centred practice frameworks.
Position Requirements
- Mental Health Nurse
Registration with the Nursing and Midwifery Board of Australia, with mental health qualifications or experience. Credentialed Mental Health Nurse highly regarded.
- Mental Health Social Worker
Accredited Mental Health Social Worker (AMHSW) with the AASW, or eligibility for it. Strength in the whole context around a diagnosis: family, housing, systems.
- Psychologist
General registration with the Psychology Board of Australia. Clinical endorsement welcome but not essential for the right person.
What You’ll Bring
- Demonstrated experience working with severe and complex mental health and psychosocial presentations, advanced clinical skills and independent judgement managing complex caseloads, and experience with risk, service coordination and multi-stakeholder involvement.
- Comfortable working autonomously while managing competing clinical priorities across rural settings, bring high-level communication and stakeholder engagement skills, and be able to rapidly adapt to working within a GP practice alongside the whole-of-practice care team.
- Demonstrated cultural competence, a strong grounding in trauma-informed and recovery-oriented practice, and a current driver's licence with willingness to undertake outreach travel and home visits are all essential.
- While not essential, applications from candidates with postgraduate mental health qualifications, experience working in rural or remote communities, expertise supporting clients with co-occurring complexities, assertive outreach experience, or knowledge of stepped-care models such as the IAR Decision Support Tool will be highly regarded.
