About the role

As a Youth Case Manager with us, you’ll find yourself in a position to deliver developmentally appropriate, youth-centered, trauma-informed care to young people aged 12–24 years. Be rewarded by the life-changing outcomes your work is supporting

About the program The role actively engages young people experiencing alcohol and other drug concerns, mental health challenges, family conflict, disengagement from education, homelessness, justice involvement, or other barriers to wellbeing. The position works collaboratively with families, carers, schools, education providers, youth services and other stakeholders to improve outcomes for young people. The aim of the Drug and Alcohol Treatment Program (DATP) is to achieve improved health and social outcomes for individuals, families, and communities at risk of, or currently affected by, problematic alcohol and other drug use.

It supports people at all stages from early intervention to relapse prevention.

This position is funded until 30 June 2027, with strong potential for extension as part of our expanding community services

Hours and location

This full-time role is 35 hours per week, Monday to Friday, with flexible work arrangements to support work-life balance. Working in a hybrid capacity, you'll split your time between our Port Macquarie office, community-based outreach, and occasional work from home. A work vehicle is provided for community appointments. This position is funded until 30 June 2027, with strong potential for extension as part of our expanding community services

$$$: This role will be remunerated at SCHCDS Level 4 $46-$50ph with access to salary packaging to potentially increase your take home pay by another 5k.

1. Tertiary qualifications at diploma level or higher in an AOD, Social work, psychology, or Mental Health field.
2. Current driver's licence, a satisfactory Criminal Record Check and Working with Children Check
3. A minimum of two years supporting young people with co-occurring mental health, AOD and psychosocial needs and experience in case management.
