About the role

The Clinical Nurse – Wound Management provides advanced clinical nursing expertise within the Post Acute Care Service (PACS), Community and Oral Health. Based at Caboolture Community Health Centre, the role delivers specialist wound management services and supports the provision of high-quality, evidence-based care for patients transitioning from acute care to community-based services.

The successful candidate will provide expert assessment, planning, implementation and evaluation of wound care interventions, while supporting consumers, families and healthcare professionals through education, leadership and clinical guidance. The role also contributes to quality improvement initiatives, professional development and the ongoing advancement of best-practice wound management services.

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

- Provide advanced clinical nursing care and wound management expertise to support positive patient outcomes within a community-based healthcare setting.
- Apply expert assessment, care planning and clinical decision-making skills to manage complex wound care presentations and changing patient needs.
- Provide education and support to patients, families and healthcare professionals to promote self-management, learning and evidence-based practice.
- Work collaboratively within multidisciplinary teams to deliver safe,



coordinated and patient-centred care across the continuum of service delivery.
- Lead and contribute to quality improvement, research and professional development activities that enhance wound management services and clinical outcomes.
