The Role

The ITC - Aboriginal Outreach and Care Coordinator works as part of the PLAHS' multidisciplinary team to improve access to culturally safe, coordinated health care for Aboriginal and Torres Strait Islander people, particularly clients with chronic and complex care needs. The position combines community outreach, practical access support and care coordination to help clients engage with primary, specialist, allied health, pharmacy and community services. The role builds trusted relationships with clients, families, community, and service providers; supports clients to understand and act on their care plans; identifies barriers to access; and contributes to practical, locally responsive solutions.

This is a full time, fixed term contract position until 30 June 2027

Essential Qualifications, Skills and Experience

- No formal qualifications required
- Must have previous experience with administrative duties
- Must have strong links with community and possess effective communication skills
- Demonstrated ability to work effectively in a multi-disciplinary team approach




- Ability to communicate effectively with both Aboriginal and non-Aboriginal people in a culturally sensitive manner
- Ability to develop, keep and maintain accurate case files and client records
- Demonstrated ability in using initiative, working independently with minimal supervision, determining priorities, meeting deadlines, planning and organising work
- Ability to effectively utilise information technology, data entry and Patient Information Recall Systems, in particular Communicare, and Microsoft Office products
- Experience in working effectively with Aboriginal and non-Aboriginal people
- Experience working in a multidisciplinary team to enhance service delivery through supporting staff expertise and morale
- Knowledge, understanding and awareness of the diversity of Aboriginal cultures and societies
- Knowledge and understanding of Cultural Safety principles and their workplace implementation
- Knowledge of Social Justice principles and practice
- Knowledge of Equal Opportunity and Work Health & Safety (WHS) Legislation
- Knowledge of relevant local resources and organisations
- Knowledge of client-centered, consultative and community development approaches

Other Requirements:

- Must hold a valid, unrestricted driver’s licence.
- A current DCSI Working with Children Check (WWCC) issued within the last 5 years.
- A National Police Certificate issued within the last three years.
- A Cardiopulmonary Resuscitation (CPR) Certificate current within 1 year
- Safe Environments for Children & Young People or approved alternative current within 3 years.
- Evidence of compliance with PLAHS staff immunisation requirements including an annual influenza vaccination.
