Position Profile: Plans, coordinates, delivers and evaluates timely and culturally appropriate Social Work services for Broomeand surrounding communities at an advanced practice level. Using a primary health care approach, contributes to the promotion, maintenance and improvement of the health and well-being of clients and families within Broome and surrounding communities. Provides clinical services for clients over the lifespan in both community and acute settings. Assists with discharge planning and coordination of care to promote optimal health outcomes for clients with complex psychosocial needs.

Provides secondary consultancy services and support to WACHS and other staff working across the region.

This is a Permanent Full Time Appointment

Suitability recognises that a range of factors contribute to merit, including skills, knowledge, abilities, behaviours, ethical alignment and relevant operational requirements. The suitability requirements below are derived from the Job Description Form and define what will be assessed to determine your suitability for the role:

Professional Qualifications and Registration - Tertiary qualification in Social Work with eligibility for full membership of the Australian Association of Social Workers (AASW) or the Society of Professional Social Workers, together with a current C Class Driver's Licence and willingness to undertake work-related travel, including overnight stays.
2. Advanced Clinical Social Work Practice - Demonstrated advanced experience, knowledge and skills in the planning, delivery and evaluation of Social Work services across primary health care, outpatient and acute care settings, working with individuals across the lifespan.
3. Primary Health Care and Clinical Leadership - Demonstrated understanding of primary health care principles and the ability to provide clinical leadership, governance and continuous quality improvement to support high-quality Social Work practice and service delivery.
4. Cultural Capability and Collaborative Leadership - Demonstrated ability to lead and work effectively within multidisciplinary and cross-cultural environments, with a strong understanding of cultural safety, social determinants of health, and the needs of Aboriginal communities.
5. Leadership, Communication and Service Management - Demonstrated leadership, communication and interpersonal skills, with experience in managing programs, teams or service delivery, building effective stakeholder relationships, and contributing to organisational objectives.
