Avenues Australia is a for-purpose Human Services organisation specialising in supporting people with highly complex and interrelated health, disability, mental health and aged-care support needs.
We provide navigational services to National Disability Insurance Scheme (NDIS) and My Aged Care Home Care Package recipients.
By staying focused on navigation and administrative services, we support our clients to effectively orientate complex and fragmented service systems.
We act as your primary service provider by helping our clients understand the system and exercise informed decision-making to enable choice and control.

About the role
The Service Coordinator role is aimed at enhancing the lives of individuals living with disabilities, mental health challenges and/or aging.
The primary responsibility of this role is to develop, implement, and monitor individualised support plans for participants with complex needs, ensuring these plans align with the legislative frameworks, to effectively meet participant goals and safeguard them.
In this role, you will support clients and their carers to effectively manage their support plans while facilitating access to community and mainstream services.
You will adopt a proactive, outcomes-focused approach that empowers clients to build capacity, increase independence, develop new skills, access high-quality care, and maintain their safety and wellbeing.
You will demonstrate the ability to:
Build strong, trusting relationships with clients, families, carers, service providers, and other stakeholders while confidently managing sensitive or challenging conversations.
Collaborate with participants, their families, carers, and service providers to develop, implement, monitor,
and adapt individualised support plans, including crisis planning and risk management where required.
Work effectively across the broader service system, particularly with mainstream services, to ensure coordinated, integrated supports that align with each participant's goals and support plan.
Key Responsibilities
As a Service Coordinator, you will:
Service
Support clients and their carers to understand and maximise their funding, goals, and support plans.
Develop, implement, monitor, and review individual support plans to achieve positive client outcomes.
Coordinate services across multiple providers, ensuring supports are responsive, effective, and aligned with client needs.
Work collaboratively with clients, carers, service providers, and mainstream services to deliver coordinated, person-centred support.
Promote client choice, control, independence, and capacity building through coaching and ongoing support.
Manage complex client needs, balancing dignity of risk with duty of care while proactively identifying and mitigating risks.
Prepare clients for plan reviews, including reviewing progress, identifying outcomes, and recommending future supports.
Coordinate assessments, funding allocations, and service arrangements to ensure supports are delivered efficiently and in line with funding requirements.
Respond to and manage crises or breakdowns in support, ensuring timely and appropriate interventions.
Maintain accurate records and deliver services in accordance with organisational policies, funding requirements, and relevant legislation
Operational
Effectively manage a complex caseload while prioritising tasks and meeting service delivery requirements.
Complete administrative duties efficiently, ensuring accurate and timely documentation.
Maintain high-quality client records that support effective service delivery and compliance requirements.
Monitor, evaluate, and report on client outcomes through clear and accurate reports that reflect individual needs and progress.
Contribute to the development, implementation, review,
and evaluation of client support programs in collaboration with clients and service providers.
Conduct risk assessments, review client goals and records, and monitor progress to support positive outcomes.
Promote Avenues Australia's Service Coordination services and build strong relationships with clients, providers, and key stakeholders.
Underpinning Knowledge
Knowledge and understanding of mental health, disability, and ageing-related conditions, including their impact on individuals and support needs.
A strong understanding of person-centred practice, including choice and control, dignity of risk, and duty of care principles.
Knowledge of mainstream, community, and specialist service providers and how they support client outcomes.
The ability and willingness to develop a detailed understanding of funding frameworks, requirements, and guidelines.
An understanding of the complexities involved in positive behaviour support and managing behaviours of concern.
Finance and Performance
Ensuring that care plans are tracked and reviewed three months prior to the end of the Plan.
Continually monitor and achieve billable time targets associated with service coordination and service delivery.
Other Duties
Conduct other duties as directed.
Qualifications
Certificate IV and above in any qualification related to Community Services, Mental Health, Occupational Therapy, Social Work or any allied health tertiary qualification or other equivalent postgraduate qualification.
Minimum of 2 years' experience in support coordination related work.
Current Driver's License and ability to undertake travel within the Perth Metro as required.
Current First Aid and CPR credentials.

NDIS Worker's Screening and/or National Police Clearance.
