Enrolled Nurse - Home Care
An prospect is available for an Enrolled Nurse to step into a Clinical Coordinator role supporting residents within a retirement living and community care environment.
The position focuses on coordinating integrated care services, ensuring residents receive the appropriate support to maintain independence, wellbeing and quality of life.
This role works closely with residents, families, care teams and external providers to coordinate clinical and community-based services, provide medical assistance and ensure compliance with aged care standards and funding programs.
Key Responsibilities
Provide holistic clinical case management for residents, promoting independence and enablement

About You
Enrolled Nurse with current AHPRA registration
Minimum 1 years experience in a similar clinical or community care role
Understanding of Support at Home and relevant funding models
Strong knowledge of community aged care programs
Experience coordinating care within retirement living, community or residential aged care settings
Ability to build strong relationships with residents, families and multidisciplinary stakeholders
Strong documentation and administrative capability
Intermediate Microsoft Office skills
Qualifications and Requirements
AHPRA Registration
National Criminal History Check
Driving License
