the program supports hospital avoidance, faster access to care and smoother transitions between primary, acute and tertiary services.
Working closely with partners across the health system, Integrated Care delivers services including Hospital in the Home, Urgent Care Hubs, rapid assessment, virtual care, aged care assessment, Refugee Health and GP integration.
