Role Responsibilities & Qualifications
Provide health promotion and education to clients and their carers
Promote and deliver our award-winning Wellness for Independence® programs, both in-person and virtually
Monitor and improve health outcomes through Telehealth.
Direct Care Services:



Provide care across various funding programs, including wound care, diabetes management, medication management, stoma care, and catheter care.
Centered Care: Assess, develop, and evaluate nursing care plans collaboratively, focusing on client goals and independence with support from Allied Health clinicians.
What you need to be considered for the role?
Understanding of community nursing and primary health care principles, including working within a consumer-directed care framework
At least 2 years of experience in community, acute, hospital or aged care
Experience working independently or as part of a team
Strong organizational skills and attention to detail
Proficiency with electronic devices and client management systems (smartphones, tablets, laptops, email, etc.)
Current Registration with AHPRA, Police check, working with children check and CPR
Experience providing advanced clinical care, including chronic disease, medication, palliative care, catheter management, stoma care, wound care, and diabetes management
