We are seeking an experienced Registered Nurse to join our Warwick Aged Care Service as a Care Coordinator, playing a key role in coordinating high-quality, person-centred clinical care for our residents.In this pivotal position, you will provide clinical leadership and oversight for residents with complex needs, ensuring care is safe, coordinated, and aligned with our Positive Wellbeing Model.
You will work closely with the Service Manager and clinical team to support service quality, compliance, and continuous improvement.About the RoleAs Care Coordinator, you will:Coordinate and oversee baseline and focused clinical assessmentsDevelop, review, and monitor individualised care plansEnsure accurate clinical documentation and AN-ACC data collectionSupport clinical programs, audits, and quality initiativesLiaise with external service providers to ensure seamless care deliveryProvide guidance and leadership to clinical staff,
