This role is ideal for a clinically experienced qualified who is ready to step into a strategic leadership role in complex care.You May Currently Be Working As aRegistered Nurse (AHPRA registered)Behaviour Support Practitioner (BSP)Allied Health ProfessionalIf you have experience supporting individuals with mental health, high-intensity supports, or complex behaviours, this role offers the opportunity to lead services that make a real difference.You will oversee safe, structured hospital-to-community transitions, ensuring individuals receive the right supports to thrive in community living.Key ResponsibilitiesLead a portfolio of clients with complex and high-intensity support needsManage hospital-to-community transitions, ensuring safe and sustainable care arrangementsBuild, mentor and lead high-performing frontline support teamsOversee care plans, behaviour support plans, and clinical risk management strategiesCollaborate with hospital discharge teams, allied health professionals, families, and stakeholdersDrive quality, compliance, and continuous service improvementBalance clinical oversight with effective portfolio managementWhat You'll BringA background in Nursing, Behaviour Support,
