Assist in planning, assessing and evaluating our clients needs with the help of our multidisciplinary team, to promote positive health outcomes.
The Clinical Care Coordinator practises independently and interdependently, assuming accountability and responsibility for their own actions and delegation of care to other Registered Nurses, Enrolled Nurses and Support Workers within program, state/territory and Calvary Home Care guidelines.
The Clinical Care Coordinator provides evidence-based nursing care to people of all ages and cultural groups, including individuals, families and communities.Key ResponsibilitiesWork with clients, their carers and the wider community to achieve identified clinical goalsPractice in accordance with Calvary and relevant Government Health policies and procedures, the position description, Code of Conduct and industrial agreementsUtilise evidenced based practice and follow Calvary Home Care policies, procedures and guidelines to provide excellent careRecognises professional, legal and ethical responsibilities which require demonstration of a satisfactory knowledge base, accountability for practice, functioning in accordance with legislation affecting nursing and health care,
