This role offers a varied caseload across aged care and disability services, with the prospect to work collaboratively alongside a multidisciplinary team and Allied Health Assistants.Caseload:Frail aged clients and people living with disabilitiesHome Care Package (HCP) clientsCommonwealth Home Support Programme (CHSP) clientsHACC PYP clientsNDIS clients (minimal exposure)Functional assessmentsHome safety assessmentsFalls prevention and risk managementAssistive technology and equipment prescriptionHome modifications and environmental assessmentsWheelchair and scooter assessmentsActivities of daily living and independence supportWhat You'll Be Doing:Conduct occupational therapy assessments within clients' homes and community settings.Provide recommendations for home modifications, assistive technology and equipment solutions.Develop personalised goals and care plans to support clients to remain independent at home.Complete functional assessments, clinical reports and funding documentation.Collaborate with clients,



families, carers and the wider Allied Health team to deliver holistic care.Work alongside Allied Health Assistants to implement therapy programs and equipment trials.Provide supervision and support to Grade 1 Occupational Therapists, students and Allied Health Assistants.Why This Role?
