About ACCORD CAREACCORD CARE provides Supported Independent Living, in‐home care, and community‐based disability supports under the NDIS.We focus on:Structured, consistent support deliveryClear care planning and clinical guidanceTrained teams working in high‐responsibility environmentsSafe, person‐centred care that maintains dignity and independenceHigh-Intensity SIL Support Role – Complex Care SettingThis is a high‐responsibility Supported Independent Living (SIL) role supporting a participant with complex physical care needs in Ferntree Gully.You will work as part of a consistent, trained support team delivering structured, hands‐on care where safety, accuracy, and calm decision‐making are essential every shift.This is not a general disability support role.
It requires experience in high physical support or complex care environments and the ability to confidently follow clinical care plans in a structured home setting.What Your Shift Will InvolveYou will be responsible for delivering safe, structured care including:Providing personal care including hygiene, grooming, and daily living supportPerforming safe manual handling and hoist transfers (2:1 support where required)Supporting mobility using assistive equipment such as wheelchairs, pressure care mattresses, and adjustable bedsAssisting with meal preparation including modified diets and dysphagia‐aware supportProviding continence care and supporting skin integrity managementCompleting pressure care routines and scheduled repositioningMonitoring participant wellbeing and escalating any changes immediatelySupporting household tasks including cleaning and laundryFollowing detailed care plans, risk management strategies, and escalation procedures at all timesCompleting accurate documentation during and after shiftsWhat Success Looks LikeConsistently delivering protected,
